Clinical indications
The principal indication for disaccharidase assay is the diagnosis of primary congenital deficiency syndromes, e.g. sucrase-isomaltase, congenital alactasia and the detection of mucosal damage after a challenge in cow’s milk sensitive enteropathy. Disaccharidase activity in a single biopsy sample is of no specific diagnostic importance in the diagnosis and management of disorders due to secondary disaccharidase deficiencies such as coeliac disease and post-gastroenteritis malabsorption. In these circumstances a depressed disaccharidase activity merely reflects the state of the mucosal damage that is present. Lactase activity appears to be more susceptible to damage than the activity of the other disaccharidases, the depression of lactase activity is usually more severe and takes longer to recover.
Specimen requirement
A minimum of 2 jejunal biopsies must be foil wrapped, placed in an empty pre-chilled polypropylene tube and snap frozen in liquid nitrogen or with dry ice immediately after collection
