Our underlying principles underpinning our model of care
Patient-centred care
This means that children, young people, and their families living with chronic endocrine conditions are involved in planning their own care, which is a critical step towards improving patient-related outcomes. We aim for a collaborative relationship between healthcare professionals and young people, and, when appropriate, we support self-management skills for young people and their carers.
Collaborative care planning
We are committed to care-planning as an active collaboration between the young person and the endocrine team in agreeing how their condition is managed. It requires us to provide sufficient information and ensuring an understanding about their condition, then jointly agreeing on priorities and actions with skilled, trained healthcare professionals. We will provide education, information resources, and involve children and young people in care pathway design. Our collaborative approach is underpinned by equitable access regardless of race, culture, language or financial means.
Multidisciplinary integrated care
Integrated care means effective communication and documentation, aiming to minimise inconsistencies and inefficiencies in our care. We aim to ensure effective shared care arrangements between our specialist services and primary, secondary, and community services. We work closely with adolescent and adult services to meet the needs of young people during their healthcare transition and transfer of care from paediatric services. Endocrine Clinical Nurse Specialists have a pivotal role in delivering high quality endocrine care, tailored to children and young people under 18 years. Our multidisciplinary team also relies on the expertise from dietitians, psychologists, pharmacists, physiotherapists, and a supported by an administrative team.
Service evaluation and clinical governance
Our continual service development is informed by up-to-date evidenced-based practice, service evaluation, and audit. We aim to involve young people, families, and carers in our service development and prospectively collect patient-reported outcome measures. We take a candid and non-judgmental approach to risk management and practise within a learning culture.
Wellbeing of our staff
We strive to support the wellbeing of our team in order to ensure they are able to provide the best care for children. We do this by listening, valuing each other's roles and kindness. We value a healthy work-life balance by discouraging "Work after Work" and instead encouraging "Home by Dinner". We are dedicated to recruiting, training and inspiring the next generation of paediatric endocrinologists and other members of the multidisciplinary team, via an appraisal process that reflects their individual personal needs and professional development so that they are successful in their chosen career paths.
